Description

Note: This is a temporary, part-time position with the ability to become permanent; temporary position will run a minimum 3 months from hire date.

Community Options has an exciting opportunity within the Behavioral Health Department in the Clinical Division as a Care Coordinator. This position works to establish working relationships with clients to develop care plans and coordinate support with other service providers by making appropriate referrals to internal and external resources and provide follow-up to ensure client's goals are met. This role is responsible for completing intakes, assessing the needs, and developing care plans to help clients overcome barriers and become independent.

The salary for this position is $18- $20 an hour depending on experience (DOE).

Qualifications

Minimum Qualifications:

  • An associate degree in a related field required, OR 1 years of experience in a setting providing direct services to clients, i.e., DSP, CNA, human services, customer service, or a related field defined by hiring manager
  • Requires a valid driver's license, acceptable driving record, reliable transportation, and proof of insurance for the vehicle subject to the statute of the licensing state and the ability to travel based on operational and clientele needs, this may include travel to outer, surrounding regions as assigned
  • Required to successfully complete the care coordination training provided by the State of North Dakota after being hired
  • Successfully pass a criminal background check, drug screening, and obtain ND Department of Human Service (DHS) pre-employment approval
  • Completion of training as required by Community Options
